Responsibilities:
Works in a team based environment to help achieve maximum benefit from our services through care integration.
Handles inbound calls from patients directly or calls transferred from case management nurse/patient advocate and perform periodic outbound calls to ensure that patients are directed to and receive care in appropriate venue in a timely manner.
Takes and records detailed history in electronic record format.
Provide remote treatment when/if condition could be managed in house while following MD24-7 policies and procedures.
May offer in-home assessment to patients with identified barriers to appropriate healthcare facilities.
Remotely monitor enrolled patients through our telemonitoring software.
Enrolls patients in our Chronic Care program and perform complete assessments for identified patients who may benefit from this program.
