Specific Description:
• Prescription Dispensing
Hospital pharmacists are responsible for accurate dispensing and timely distribution of drugs and medicines for in-patients. They are also responsible for checking prescriptions to ensure that there are no errors and that they are appropriate and safe for the individual patient.They work to ensure that the best treatment option has been chosen, working with healthcare professionals to create the most effective treatment.
• Patient Care and Treatment
Hospital pharmacists are required to go around the ward on rounds with or without the doctor to ensure an active role in the treatment process in terms of drug prescription. This includes highlighting a drug’s potential side effects, identifying harmful interactions with other drugs and assessing the suitability of treatments for patients with particular health conditions. The hospital pharmacist is also required to take patient drug histories and involvement in decision-making on appropriate treatments.
• Patient and Physician Liaison
The hospital pharmacist must liaise with the physicians and nurses and any other health care professional involved with the patient care to create the drug treatment and to ensure the delivery of safe, effective and economic drug treatment.
• Patient Counseling
Then the pharmacist must counsel and educate the patients on the effect, dosage and administration. They have to ensure that the patient understands the drug, its side effects and any possible interactions with other drugs and diet.
• Quality and Standard of Drugs
Preparing and quality-checking sterile medications, for example intravenous medications. The pharmacist has to ensure that the medications are all given under sterile conditions and that the storage is as per drug container specifications. The medication must be checked to ensure the proper dosage is being provided and that the form is appropriate as well (e.g., cream, tablet, inhaler).
